North Korea Promotes Community Doctor System but Residents Report Poor Care

North Korean authorities are promoting the community doctor system as a preventive healthcare pillar in 2026. Residents across the country report that the initiative amounts to little more than routine hygiene lectures. Medical workers in Kaechon city have made rounds of factories, schools, and neighborhood units. They deliver education on boiling water and frequent handwashing. The focus includes respiratory illnesses, skin conditions, and seasonal infectious diseases.

This community doctor system assigns medical workers to designated geographic zones. Doctors must monitor the health of everyone in their assigned area. Alongside hygiene lectures, medical workers conduct detailed health surveys. They catalog which illnesses people have and whether patients receive treatment. Medical workers describe this data collection as foundational work for a community health management system.

The response among Kaechon residents has been largely skeptical. A local source said repeating hygiene propaganda does little good when clinics lack proper treatment. Most clinics in Kaechon lack basic medical equipment and emergency supplies. Each clinic operates with one or two doctors and three or four nurses. The main services are vaccinations and administering privately obtained injections. Therefore even when a doctor visits, meaningful medical intervention remains out of reach.

The community doctor system has also generated unease about frequent health checks. Some residents feel burdened by constant monitoring of personal health information. Complaints describe the system as all supervision and no help. Many call for the state to prioritize medicine supply over data collection. Not all reactions have been negative, however. Some residents note that medical workers are now far more visible than in the past. A degree of cautious hope exists that the community doctor system could eventually translate into tangible improvements. People hope this activity leads to proper treatment when sick.
